Faculty Grading

The Mid-Semester Exam period begins on

• Week 7 for Fall
• Week 7 for Spring  
• Week 5 for Summer

An Academic Warning Notice should be given to any student who falls into one or more of the following categories:

• has below a “C” (2.0) average
• has missed significant amounts of work
• has missed two or more classes, or has excessive tardies
• has never attended class
• other reasons which you deem appropriate

You may obtain Academic Warning Notices from either your Department Office or the Office of Registration and Records.

Please complete this form in ink, including ALL student and course information, and distribute in the following manner:

• White - Office of Registration and Records
• Yellow - Department Office
• Pink - Student*
  * If for any reason you cannot contact the student, please notify your Department Office and
  they will mail the student his/her copy.

Please inform your students of the withdrawal deadline as outlined in the Academic Calendar.
